CMHA Registration Forms

Please download the appropriate registration form below (option 1 or 2 below) and fill-in the required information. Please forward two (2) copies of the completed form, along with the appropriate fees payable to Coronation Minor Hockey Association to the address on the registration information page. CMHA is not responsible for registration forms or cheques lost or stolen in transit. Full registration will only be credited once payment is received. Refunds and NSF cheques are subject to an administrative charge.

Option 1:
To type in the required information using MS-Word please download the following file (HMHC registration form.doc), open it in MS-Word and type in the appropriate information by tabbing between the fields. Once complete, please save the file for your records then print off and sign two (2) copies of the registration. 

Option 2:
To fill in the form using a pen, please download the following file (HMHC registration form.pdf) and print three (3) copies of it.

PLEASE NOTE:
Coronation Minor Hockey is obliged (no option) by Alliance Hockey to pass on to them all registration information including Full Names, Dates of Birth and Addresses for all players and coaches. Alliance Hockey and the Hamilton Minor Hockey Council has been made aware of the fact that Coronation Minor Hockey objects to this transfer of information due in part to the way the information is stored and the way it is managed on privacy and information security grounds. Parents are advised to read the following Privacy Policy which outlines the use of this information by Alliance Hockey (privacy policy).
